The Case Manager RN Home Health WKND plans, coordinates, and manages the care provided to assigned patients.
This position acts as the leader in collaborating with members of the interdisciplinary team, providing the foundation for creating the interdisciplinary plan of care.
The Case Manager coordinates the various care processes that are necessary to achieve desired outcomes, and delegates, as appropriate.
The Case Manager works to promote and sustain a culture reflective of Centra's best practices for quality and service, creates and maintains a safe patient environment, and contributes to a healthy work environment.
Qualifications/Education/Licensure:
Graduate of a school of nursing.
Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse.
Two years nursing experience.
Prefer two years of acute care clinical experience, critical care or emergency room experience.
Recommended Skills Certified Nurse Practitioner Clinical Works Critical Care Medical Emergency Nursing Estimated Salary: $20 to $28 per hour based on qualifications.
